Submitting Your Immunization Records
All incoming undergraduate and graduate students must submit proof of immunizations prior to their arrival on campus. Use the two-step process outlined on this page to submit your information.
- PRINT and TAKE the Immunization Verification Form to your health care provider for completion.
The Immunization Verification Form MUST contain the required immunization information along with the provider's signature, title, date, address and ORGANIZATIONAL STAMP.
